on an entity form need to create a popup

Hi Group,

I created a portal entity form and need to do the following:

1-to put a button on the form labeled "Add Comment".

2-Then when user clicks the button I need a popup to show text box to add comment and attachment browse control to add files against the entity,

Can anyone help on this 2 questions?
