MVC4 Web App & CRM 2015 Integration

Hi House,

I've Built a Web App in MVC 4 that capture some Details From the User, There is a need for me to Integrate this Application with Dynamics CRM 2015 as the BACK END, where All the Entries on the Form Should be Sent To CRM once the SUBMIT Button is Clicked.
These Records should be able to create an account that can be tracked and Managed in CRM 2015.

Kindly Assist with the Most Possible Guide in getting this done.
