I am on CRM 2016 On Premise and have created a new Organization without any error. When I try to access, I receive a 404 error. I have one other Organization that I can access without any issue. I have claims based and IFD configured and only can access via HTTPS. I disabled\deleted the Organization and tried creating again, same error. I have custom AD accounts running all CRM services, I do not use system\network service accounts.
