Stakeholder sub-grid issue

On our Opportunity Form, we have both the Stakeholders and Sales Team grids. We have an intermittent problem of the "Role" not taking when adding a Stakeholder inline.

It doesn't matter what Stakeholder role is selected. To the user it appears that the stakeholder was added correctly, but if they leave the form and go back to it their stakeholder has disappeared. We're able to find the stakeholder by looking at the list of Connections for the opportunity, but we'll see that this stakeholder does not have a role assigned (and thus wouldn't show up in the Stakeholders sub-grid).

We haven't touched the "Stakeholder" connection role category. But we have inactivated and reactivated the individual stakeholder connection roles while trying to fix the issue.

The really weird thing, is that this seems to work correctly roughly 25% of the time. I see the same behavior with System Admin privileges. I can take the same steps repeatedly, and sometimes a role is assigned, otherwise the role appears to be assigned, but isn't actually set.

Has anyone else seen this behavior? Any ideas on what else I could try to fix the issue?
