When attempting to close an opportunity as 'Close as Won' or 'Close as Lost' via the buttons at the top of the form nothing happens - the window opens where you provide a status reason but when you click ok the window closes and no action is performed, no error message either. The opportunity remains open.
The only way I have been able to close an opportunity is to create a yes/no option field which, when yes is selected, changes the status to complete.
A similar issue is occurring when attempting to mark an activity as 'Completed'.
Does anyone have any suggestions on how to fix this issue?

Issue occurs using IE/Chrome/Opera and is not caused by business processes or business rules.
Also happens to all users including system administrators.
